You don't need to have everything — Emi can work with what you have. But the following information helps:
- The person's name, age, and location
- A brief description of their continence concerns
- Current care arrangements and any relevant medical history
- NDIS plan details or funding source (if applicable)
- Your contact details and preferred communication method

How the Referral Process Works
Get in touch
Call or email Emi with basic details about the person you're referring — their name, location, and the nature of their continence needs.
Referral discussion
Emi will discuss the referral with you, confirm the most appropriate service, and answer any questions you have about the process.
Consent & scheduling
With consent from the individual (or their representative), an assessment or education session is scheduled at a time that suits.
Service delivery
Emi delivers the assessment or education session, then provides a written report and follows up with the care team as needed.

All client information shared with Empower Continence Consulting is handled in accordance with the Australian Privacy Act 1988 and relevant health privacy legislation. Information is used only for the purpose of delivering the requested service and is not shared without consent.
